Family Health Insurance for Self-Employed Parents: One PPO for Everyone
One private PPO for you, your spouse and your kids, with a set yearly limit so one ER visit doesn’t wreck the year.
The best family health insurance for self-employed parents is often one private PPO that covers the whole family, with a deductible you choose and a set yearly maximum out of pocket.
I help self-employed families get a private, off-exchange PPO with a broad national network, no referrals for specialists, and first-dollar benefits for office visits, labs and prescriptions. You can enroll any time of year, and coverage often starts as soon as next month.
What your family gets
- One PPO for the whole family
- First-dollar office visits, labs and prescriptions
- Your deductible, $2,500 to $10,000
- A set yearly maximum out of pocket
- Optional accident protection
Why Is Family Health Insurance Hard When You’re Self-Employed?
You run the business. You also have to cover the whole family, with no HR team to help.
No employer plan to join
You own the business, so you pick and buy the family’s coverage yourself.
Kids need the doctor a lot
Ear infections, fevers, refills. On many plans that care waits behind a high deductible.
One ER visit can wreck the budget
Kids fall and spike fevers at midnight. Without a good cap, every ER trip is a big bill.
Narrow networks and referrals
Many Marketplace plans are HMO or EPO plans with smaller networks.
Families don’t stay in one place
Vacations, grandparents out of state, college one day. Your plan should travel too.
Waiting for Open Enrollment
Marketplace plans open once a year unless you have a qualifying event.
What If the Kids’ Checkups Are Fine but the ER Bills Aren’t?
A family I’m working with this month: a young couple who own an LLC, with three small kids, on a Marketplace silver plan.
The kids’ checkups were fine. But every ER visit turned into a $5,000 to $10,000 bill. With three little ones, that adds up.
What I’m showing them: one family PPO where the kids’ everyday care (office visits, labs, prescriptions) has first-dollar benefits, the family picks the deductible that fits the budget, and a set yearly maximum out of pocket puts a ceiling on a bad year. They can also add accident protection for the kids.
Sound familiar? I’ll compare it with what you have now.
Marketplace Plan or One Family PPO?
The question self-employed parents ask me most.
What many families end up with
- Often an HMO or EPO with a narrower network
- High deductibles before most care is paid
- Sign up only in Open Enrollment or with a qualifying event
- Checkups covered, but ER visits can still mean big bills
One plan built around your family
- One PPO for you, your spouse and your kids
- First-dollar benefits for office visits, labs and prescriptions
- A deductible you choose and a set yearly maximum
- Broad national network, no referrals, enroll any month
If your family is healthy, one private PPO often fits better. The kids’ everyday care is easier to use, and the yearly maximum keeps an ER visit from taking over your year.
What Are the Health Insurance Options for a Self-Employed Family?
How the main paths compare for a family with kids.
| Option | Network | Kids’ everyday care | ER and big bills | Enrollment |
|---|---|---|---|---|
| Private family PPO | Broad national PPO, no referrals | First-dollar benefits for office visits, labs, x-rays, prescriptions | Deductible you choose, 80/20, set yearly max | Any time of year; health questions and approval |
| Marketplace plan | Often HMO/EPO, narrower and state-based | Checkups covered; other care depends on the plan | Often a high deductible first | Open Enrollment or a qualifying event |
| Spouse’s employer plan (adding the whole family often gets expensive) | Varies by employer | Varies | Varies | Employer windows |
| Short-term plan | Varies | Limited | Limited | Temporary stopgap only |
How Does Family PPO Health Insurance Work?
A private family PPO is bought outside Healthcare.gov, directly through a licensed agent like me.
It uses broad national PPO networks, the same kind of networks large employers use. You can use any network hospital and most doctors across the country, with no referrals for specialists.
You pick the deductible, from $2,500 up to $10,000. After that, the plan pays 80% and you pay 20% until you reach your set yearly maximum out of pocket. There is no lifetime maximum. For a deeper look at all your choices, read the full guide for self-employed people.
Why a Family PPO Works for Self-Employed Parents
Easy everyday care. Protection for bad days.
One plan for everyone
You, your spouse and the kids on one PPO.
Everyday care first
First-dollar benefits for office visits, x-rays, labs and prescriptions.
A ceiling on a bad year
A set yearly maximum out of pocket caps a bad year.
A deductible that fits
Choose from $2,500 up to $10,000.
Accident protection option
Add it for the falls and breaks that come with kids.
A national network
For family trips now and college later. Kids can usually stay on the plan until 26, depending on your state.
How Much Is Health Insurance for a Self-Employed Family of 4?
The short answer: the price of a private family PPO depends on everyone’s age, your ZIP code and the deductible you pick.
What moves your price:
- The deductible you choose. Higher deductible, lower monthly cost.
- Who is on the plan. Each person’s age counts.
- Add-ons. Accident or critical-illness coverage add to the price.
When Can a Self-Employed Family Sign Up?
You can apply for a private family PPO any time of year. Once you’re approved, coverage often starts as soon as next month.
Marketplace plans work differently. Open Enrollment for 2027 runs November 1, 2026 to January 15, 2027. Outside it, you need a qualifying life event. See the Open Enrollment dates for details.
Private PPO plans are medically underwritten: your family answers health questions and the plan has to approve you. Not everyone qualifies. Details are in the FAQ below.
Who Is a Family PPO For?
