PPO Health Insurance for Individuals and Families: A Real PPO You Can Own, Any Time of Year
PPO health insurance is a plan that lets you see doctors and specialists in a large network without a referral. If you buy your own coverage, you can still get a private PPO with a broad national network, a deductible you choose, and enrollment any time of year.
Can’t find a PPO on Healthcare.gov? You can still own one.
In many areas, Marketplace plans are mostly HMOs and EPOs with narrow, local networks and high deductibles. I help individuals and families get a private, off-exchange PPO instead: no referrals, the same kind of networks large employers use, and coverage that can often start as soon as next month.
- See specialists with no referral
- Broad national PPO network
- Pick your deductible, from $2,500 to $10,000
- 80/20 coinsurance and a set yearly out-of-pocket maximum
- Unlimited lifetime plan maximum
- Enroll any month, not just Open Enrollment
Why Are There No PPO Plans on Healthcare.gov in Many Areas?
Mostly HMO and EPO plans
In many areas, Marketplace PPO choices are few or gone, leaving plans with a smaller list of doctors.
Narrow, local networks
Your doctor may not be in it, and care outside the network is usually covered only for emergencies.
Referrals before specialists
With an HMO, you often need a primary care referral before you see a specialist.
High deductibles you didn’t pick
Many plans come with a high deductible set by the plan, not by you.
A short window to sign up
Without a qualifying life event, you wait for Open Enrollment.
Marketplace HMO or Private PPO: Which Fits You Better?
An HMO or EPO with a local network
- Smaller network, often limited to your area
- Referrals often needed to see a specialist
- Deductible set by the plan
- Sign up only during Open Enrollment or after a life event
A PPO you own, built your way
- Broad national PPO network
- See specialists with no referral
- You choose your deductible, $2,500 to $10,000
- Enroll any time of year
- Price based on age, ZIP code and deductible, not income
If you want to keep your doctors and see specialists on your own terms, a private PPO is often the better fit. A quote shows you real prices for each deductible.
HMO vs PPO Health Insurance: What’s the Difference?
| Plan type | Specialist referrals | Network | When you can enroll | Deductible |
|---|---|---|---|---|
| HMO | Usually required from your primary care doctor | Local; out-of-network care usually covered only in emergencies | Open Enrollment or a life event | Set by the plan |
| EPO | Often not required | Local; out-of-network care usually covered only in emergencies | Open Enrollment or a life event | Set by the plan |
| Marketplace PPO (where offered) | Not required | Larger network; often some out-of-network coverage at a higher cost | Open Enrollment or a life event | Set by the plan |
| Private PPO through me | Not required | Broad national PPO network | Any time of year | You choose, $2,500 to $10,000 |

How Does a Private PPO Plan Work?
A private PPO is individual PPO health insurance you buy outside Healthcare.gov, through a licensed agent like me. It is the private alternative when you can’t find a PPO option on the Marketplace.
You get a broad national PPO network, the same kind of networks large employers use. That means access to any network hospital and most doctors across the country, and you can see specialists without a referral.
You choose your deductible, from $2,500 up to $10,000. After the deductible, you pay 20% and the plan pays 80%, up to a set yearly out-of-pocket maximum. There is no lifetime cap on what the plan pays.
With the everyday-care rider (available in most states), you also get first-dollar benefits (no deductible) for doctor office visits, x-rays, lab work, prescriptions, and wellness and screenings. You can add optional accident protection and critical-illness coverage that pays a lump sum.
Why People Choose a Private PPO Over a Marketplace Plan
Keep your doctors
A broad national network makes it more likely the doctors you already see are in it.
No referrals
Go straight to a specialist when you need one.
Covered across the country
Network hospitals and most doctors nationwide, at home or on the road.
Your deductible, your call
Pick from $2,500 to $10,000 to balance your monthly cost and your risk.
Everyday care before the deductible
Office visits, labs, x-rays and prescriptions through the everyday-care rider (available in most states).
Enroll any time of year
Start when you need it, often as soon as next month.
How Much Does PPO Health Insurance Cost?
The short answer: the price of a private PPO depends on your age, your ZIP code and the deductible you pick. It does not depend on your income.
Looking for affordable PPO health insurance? These are the levers you control:
- Your deductible. A higher deductible lowers your monthly cost. A lower one raises it.
- The everyday-care rider. It covers everyday care like office visits and prescriptions before the deductible.
The best PPO health insurance for you is the one that keeps your doctors in network at a monthly cost you are happy with. The fastest way to see real numbers is a quote.
When Can You Enroll in a PPO Plan?
Any time of year. My private PPO plans don’t wait for Open Enrollment. Once you are approved, coverage can often start as soon as next month.
Marketplace Open Enrollment for 2027 runs from November 1, 2026 to January 15, 2027. Outside that window, you need a qualifying life event to start a Marketplace plan. See all the Open Enrollment dates.
Private PPO plans are medically underwritten. You answer health questions and the insurance company approves you, so not everyone qualifies. Pre-existing conditions you list on the application are reviewed in underwriting. Conditions you don’t list have a 12-month waiting period. Once you are approved, your plan can’t be cancelled because your health changes, as long as you answered truthfully.
Who Is PPO Health Insurance For?
A private PPO is a strong fit for healthy people who buy their own coverage and want more choice.
Private PPO plans are health-based, so a short call with me tells you if it’s a match.
