For Early Retirees Before Medicare

Health Insurance for Early Retirees: Keep PPO Coverage Until Medicare at 65

You’re used to a PPO through work. You can keep that kind of coverage after you retire, with a plan you own.

If you retire before 65, you need your own health insurance until Medicare starts. Many early retirees choose a private PPO that works like the plan they had at work: a broad national network, no referrals, and a deductible they choose.

You can enroll the month you need it, so you don’t have to wait for Open Enrollment. Once you’re approved, your coverage can’t be cancelled because your health changes.

What you can keep after you leave work

  • A PPO network across the country
  • Your doctors, when they’re in the network
  • A deductible you pick, from $2,500 to $10,000
The Reality

What Changes When You Retire Before 65?

Retiring early is a big win. Health insurance is the part that needs a plan.

🏢

Your work plan ends

When you leave, your employer’s coverage stops. Now the choice is yours.

💳

COBRA costs the full price

Same plan, but now you pay the whole cost, including your employer’s share.

⏳

COBRA runs out before 65

Federal COBRA usually lasts 18 months. Retire at 60 and that leaves years before Medicare.

🏥

Marketplace plans feel smaller

Most are HMOs or EPOs with high deductibles and narrow networks.

👫

One of you retires first

Your spouse may still be working. You need a plan that fits you both.

🌴

You split time between two states

A local HMO doesn’t travel. Snowbirds need a network that works in both homes.

The Core Decision

COBRA or Your Own PPO: Which Is Better Before Medicare?

The question I hear most from people leaving work early.

COBRA

Your old work plan, at full price

  • Same plan, but you pay the full cost
  • Ends after a set time, usually 18 months
  • You can’t change the deductible or network
VS
Your Own Private PPO

A plan you own until Medicare

  • Broad national PPO network, like the plans large employers use
  • See specialists without referrals
  • Choose your deductible, from $2,500 to $10,000
  • Enroll the month you need it
  • Can’t be cancelled because your health changes
My Advice for Healthy Early Retirees

Before you sign up for COBRA, get a private PPO quote and compare. If your health may not pass underwriting, COBRA can be the safer bridge, and I’ll tell you that.

What Is Bridge Health Insurance Before Medicare?

Bridge health insurance, or retirement health insurance before Medicare, covers you from the day your work coverage ends until Medicare starts at 65. The one I offer is a private, off-exchange PPO, bought through a licensed agent, not on Healthcare.gov.

It uses broad national PPO networks, the same kind of networks large employers use. You pick a deductible from $2,500 to $10,000, then the plan pays 80% and you pay 20% up to a set yearly maximum out of pocket. There’s no lifetime maximum.

With the everyday-care rider (available in most states), doctor visits, x-rays, lab work, prescriptions and screenings are covered before you meet the deductible. You can add accident and critical-illness coverage.

Good to know: this plan is medically underwritten, so not everyone qualifies. Conditions you list on the application are reviewed in underwriting. Conditions you don’t list have a 12-month wait.

The PPO Advantage

Why Early Retirees Choose a Private PPO

It feels like your work plan, without the job.

🌐

A national PPO network

Broad access across the country.

🩺

No referrals

See a specialist without asking first.

👩‍⚕️

Keep your doctors

If they’re in the network, you keep them. I check first.

🎚

Choose your deductible

From $2,500 to $10,000.

📅

Enroll the month you need it

Often starting as soon as next month.

🔒

Coverage you can count on

Once approved, it can’t be cancelled because your health changes.

No Gaps Before 65

What Happens When COBRA Ends Before You Turn 65?

COBRA usually ends before Medicare begins. Your own PPO keeps going.

Step 1
You leave work
Step 2
COBRA ends (usually 18 months)
Step 3
You turn 65: Medicare starts
COBRA: covers you for a set time, then ends Your private PPO: covers you all the way to Medicare

Your private PPO belongs to you, not a former employer. No countdown clock. One plan that carries you to 65.

What It Costs

How Much Does Health Insurance Cost Before Medicare?

The short answer: a private PPO is priced by your age, your ZIP code and the deductible you pick.

How to keep it affordable:

  • Your deductible. A higher deductible lowers your monthly cost. A lower one raises it.
  • The extras you add. Accident and critical-illness coverage are optional.

Couples can be quoted together. Then compare it with COBRA.

When You Can Enroll

How Do You Get Health Insurance Before Medicare?

You can apply for a private PPO any month of the year. Once you’re approved, coverage often starts as soon as next month.

  • Get a quote with your age, ZIP code and deductible.
  • Check your doctors. I help you confirm your doctors are in the network.
  • Answer the health questions for approval.
  • Pick your start date to line up with your last day of work coverage.

Marketplace Open Enrollment for 2027 runs November 1, 2026 to January 15, 2027. Outside it, you need a qualifying event. See the open enrollment dates for details.

Health Insurance for Early Retirees: FAQ

Get a quote, check that your doctors are in the network, answer the health questions, and pick a start date that matches the day your work coverage ends. You can apply for a private PPO any month, and coverage often starts as soon as next month after approval.

For healthy early retirees used to a PPO at work, a private PPO is usually the closest match: a broad national network, no referrals, and a deductible you choose. If you have conditions that may not pass underwriting, COBRA or a Marketplace plan may fit better.

COBRA keeps your same plan, but you pay the full cost and it ends after a set time. A private PPO is a plan you own until Medicare. If you’re healthy, compare a private quote with your COBRA cost first.

Federal COBRA usually lasts 18 months. If you retire several years before 65, it often ends before Medicare starts.

A private PPO is priced by your age, your ZIP code and the deductible you pick, from $2,500 to $10,000. A quote gives you your real number.

These plans are medically underwritten, so not everyone qualifies. Conditions you list on the application are reviewed in underwriting. Conditions you don’t list have a 12-month wait. Once approved, your coverage can’t be cancelled because your health changes.

Yes. A broad national PPO network works in both places, so snowbirds aren’t stuck with a local HMO in one home.
